Every guide I've read suggests that the first 3 points be put into run speed; after that opinions differ. I can say that when sow drops I don't notice much difference since I have run3.

Yes, the titles begin after you have six AA points. They differ depending on what class you are and how many points you have. I haven't paid much attention to that. Minalcar usually beats me to it so I just follow him and ask his advice. :)

Note on titles :
Level titles replace the class name, like "Bard" on the /who list
AA titles are pre-pended to your name, ie Baron Irodanis (just as an example, heh)

For recommended points placement, it will really vary by class and your obejctives for your characters. Example: Some bards like to tank a bit, some prefer to be weenies and sit in the back chanting DoTs. So that will help you decide where to place points. Also, if you're a little weak in one area, AAs can help make up for some of that by adding additional points in STA, STR, or resists ... etc etc

Rangers are also encouraged to spend the first points in Runspeed.

I have seen a number of screenshots showing crits on a manaburn.

That aside, 2 handed bash is awesome for us hybrids that will never use a 1 handed weapon again.

And then there's the super horse! That's the ultimate goal of any pally/sk.
